GLP-1 medicines in India: semaglutide and tirzepatide explained
Medically reviewed by Dr. Om J Lakhani on 18 Oct 2026 · Updated 18 Oct 2026
Short answer
GLP-1 medicines such as semaglutide and tirzepatide copy gut hormones that reduce appetite and help the body handle sugar, and in trials they led to much larger weight loss than older medicines. In India, tirzepatide has been sold since 2025, and semaglutide became much cheaper after generic injections launched when its patent expired in March 2026. They are prescription medicines for people who meet medical criteria, are usually long term, and must be started and adjusted by a doctor.
Few medicines have created as much excitement, and as much confusion, as the “weight-loss injections”. Friends, social media and pharmacies all have opinions. This page explains, in plain words, what these medicines are, how they work, who they are meant for, and what the situation in India looks like as of October 2026. Prices and brands are changing quickly, so treat the availability details as a snapshot and check with your doctor.
What are GLP-1 medicines?
GLP-1 (glucagon-like peptide-1) is a hormone your gut releases after a meal. It tells the brain you are full, slows how fast the stomach empties, and helps the pancreas release insulin when sugar is high.
GLP-1 medicines copy this hormone, but they last much longer in the body. The two you will hear about most in India are:
- Semaglutide. A GLP-1 medicine. Sold by Novo Nordisk as Wegovy (for weight management) and Ozempic (for type 2 diabetes) injections, and as Rybelsus tablets for type 2 diabetes. Since March 2026, many Indian companies also sell generic semaglutide injections.
- Tirzepatide. A “dual” medicine that acts on both GLP-1 and a second gut hormone called GIP. Sold by Eli Lilly as Mounjaro, and by Cipla under the brand Yurpeak.
Older medicines in the same family, such as liraglutide, exist too, but semaglutide and tirzepatide are now the most used.
How do they help with weight?
They work mainly on the brain’s appetite centres. People typically notice:
- feeling full sooner and for longer
- fewer cravings and less “food noise”
- smaller portions without the constant struggle
Slower stomach emptying adds to this, at least in the early months. They also lower blood sugar, which is why they are widely used in type 2 diabetes. Semaglutide has also been shown in a large trial to lower the risk of heart attacks and strokes in people with heart disease and excess weight.
How much weight loss can people expect?
In the main clinical trials, alongside diet and activity advice:
- In the STEP 1 trial, adults with obesity on weekly semaglutide lost about 15 percent of their body weight on average over about 16 months, compared with about 2 percent on placebo.
- In the SURMOUNT-1 trial, adults on weekly tirzepatide lost up to about 20 percent of body weight on average over 72 weeks at the higher strengths.
These are averages. Some people lose much more and some much less. Results in everyday life are often lower than in trials, because people stop early, miss weeks or cannot reach the target strength. People with type 2 diabetes usually lose a little less weight.
Who are these medicines for?
They are prescription medicines for people with obesity, or overweight with weight-related health problems, who meet medical criteria. Indian guidelines use lower BMI thresholds than Western ones. They are not cosmetic medicines for losing a few kilos before a wedding. Read who should consider weight-loss medicines.
They are not suitable in pregnancy, and some people should not take them at all, such as those with a personal or family history of medullary thyroid cancer.
Injection or tablet?
- Injections. Wegovy, Ozempic, generic semaglutide, Mounjaro and Yurpeak are once-weekly injections under the skin of the belly, thigh or upper arm. They come as pre-filled pens or vials. The needle is very fine, and most people find it far easier than they expected.
- Tablets. Oral semaglutide (Rybelsus) is available in India, but it is approved here for type 2 diabetes, not for weight management. It must be taken in a specific way on an empty stomach. In December 2025 a higher-strength semaglutide pill for weight was approved in the USA, but as of the latest information we found, it had not been launched in India. Other weight-loss pills are in late-stage trials.
Your doctor will help choose the form that suits you.
What does availability and cost look like in India in 2026?
This is the part that is changing fastest. As of October 2026:
- Tirzepatide was launched in India by Lilly in March 2025, with the KwikPen approved later that year, and Cipla began selling it as Yurpeak in December 2025. Generic tirzepatide was not part of the 2026 semaglutide launches.
- Semaglutide for weight (Wegovy) was launched in India in June 2025.
- Semaglutide’s patent expired in India in March 2026. Several Indian companies, including Sun Pharma, Dr Reddy’s, Zydus, Glenmark, Natco and others, launched generic semaglutide injections soon after. Novo Nordisk then announced price cuts for its own brands from April 2026.
- Monthly costs vary widely by company and strength. At launch, many generic semaglutide options were reported at roughly one to a few thousand rupees a month, much less than the original brands cost before the patent expired.
Because these are long-term treatments, cost over many months matters. Your doctor can discuss options that fit your budget. Medicines bought online without a prescription, or “compounded” versions of unknown origin, can be unsafe. Buy only from licensed pharmacies, with a prescription.
Are they safe?
For most people, yes, when prescribed and monitored properly. The most common side effects are nausea, constipation, diarrhoea, and reflux, especially in the first weeks and after each step up. Less common but important risks include gallstones and pancreatitis. Read GLP-1 side effects and how doctors manage them.
Fast weight loss can also mean muscle loss, so protein and strength exercise matter. Read protein and muscle on weight-loss medicines.
Do I have to take them forever?
Obesity is a chronic condition, and in trials most people regained much of their weight within a year of stopping. Many people therefore need long-term treatment, but plans differ. Read stopping GLP-1 medicines and weight regain.
What should you ask your doctor?
- Do I meet the criteria for a weight-loss medicine?
- Semaglutide or tirzepatide: which suits me, and why?
- What side effects should I expect, and how will we handle them?
- What will this cost over a year, and what happens when I stop?
Starting one of these medicines is a medical decision best made with a specialist. Book a consultation.
